Important Update About Wellbeing Network Referrals

The City & Hackney Wellbeing Network is in its final contract year. The current funding is due to end on 30 June 2027.

We expect to stop accepting new referrals from January 2027, or earlier if capacity is reached.

We are no longer accepting re-referrals from people who have already been discharged from the Wellbeing Network.

We will share updates about any future funding or services as soon as we know more. There will also be opportunities for feedback and discussion during the wind-down.

The service is for people living in the City of London or Hackney.

The Wellbeing Network works in English. Derman can support Turkish-speaking residents. Bikur Cholim can support Hebrew-speaking residents. If you need support to complete this form, please contact one of the Wellbeing Network partner organisations of your choice.

You are making a Shared Care referral to the Wellbeing Network (WBN).

This means you will keep working with the person you are referring, even after they start getting support from us.

By making this referral, you agree to:

Complete the assessment with the person before they start accessing the WBN.
Support them to register interest in WBN courses (via our website).
Complete a review at three months.
Complete an exit review at nine months, or at any point when the person decides to exit the service.

Shared Care means that key partners can support people to access the WBN as part of their existing support plan.

Queer Connect - a fortnightly, young LGBTQIA+ online space to connect, share and explore mental health and wellbeing (ages 18 to 25).

Radical Self-Care courses (RadSec)- An 8 Week online group course teaching self-care and mindfulness skills tailored to the unique mental health needs and experiences of LGBTQIA+ individuals, to help build self-acceptance and resilience.

Move with Pride - free movement and wellbeing sessions, led by and for LGBTQIA+ Londoners. Activities include mindful walks, running clubs, open water swimming, yoga, Boxing & Mindfulness, and more .
