The City of London wants to hear what you think of pharmacy services in the Square Mile, to help them develop services in the future. Your views will help inform their Pharmaceutical Needs Assessment (PNA) which will look at health needs in the City of London, the level and accessibility of pharmacy services and how these will be maintained and developed in the future

The questionnaire should take no longer than five minutes to complete. The information in the questionnaire you provide is confidential. Please be honest with your answers so we they accurately assess areas where pharmacies are already performing well and areas that need improvement.

Paper copies of the questionnaires are also available from GP surgeries, pharmacies, and libraries across the Square Mile. To request a paper copy please call the Public Health team on 020 8356 7100 (Monday to Friday 9am-5pm).

The deadline for completion is midnight on Friday 10 December 2021. They will use your feedback to help develop our draft of the Pharmaceutical Needs Assessment which will then be available for public consultation in 2022
